Welcome to the OTOWA Forms Page.  On this page you will find the forms need to organize and run a tug of war event.  To print a form, just click on the appropriate link to load it on your computer screen.  Then click file, followed by print.  The forms are setup to be printed on letter size (8 1/2 X 11) paper. Waiver Release form

Form to be signed by each competitor waiving liability of event in case of injury

Weigh-in Form

Form with team lineup for any division in order to be weighed in order to compete

Score Sheet Form

Form to keep track of results during competition

Draw Form

 Form with schedule of pulls for an event of 2 to 8 teams

School Event Guidelines

 Guidelines for running a successful school tug of war event
